If the Paraneoplastic Ab Screen, IFA is positive for Aquaporin-4 Ab, IFA, then Aquaporin-4 (AQP4) Antibody (IgG), CBA, CSF will be performed at an additional charge (CPT code(s) 86255). If Aquaporin-4 (AQP4) Antibody (IgG), CBA result is positive, then a titer will be performed at an additional charge (CPT code(s) 86256) . If the Paraneoplastic Ab Screen, IFA is positive for ANNA3 Ab IFA, then a titer will be performed at an additional charge (CPT code(s) 86256) . If the Paraneoplastic Ab Screen, IFA is positive for PCA2 Ab IFA, then a titer will be performed at an additional charge (CPT code(s) 86256). If the Paraneoplastic Ab Screen, IFA is indeterminate or positive for ANNA1, ANNA2, PCA1, PCA-Tr (DNER), Amphiphysin, GAD65, or CRMP5 (CV2) antibodies, then Paraneoplastic Antibody, Line Blot will be performed at an additional charge (CPT code(s) 84182 x7, 86341). If the Paraneoplastic Ab Screen, IFA is positive for NMDAR1, LGI-1, CASPR2, AMPAR1, AMPAR2, GABA receptor B, or Voltage Gated Antibodies, then Paraneoplastic Antibody, CBA IFA will be performed at an additional charge (CPT code(s) 86255 x6).
