Fasting specimen preferred.

SPECIMEN PROCESSING
Specimen processing instructions:
Allow serum to clot. Centrifuge to separate serum or plasma within 2 hours of collection. 
Transport temperature:
Refrigerated (preferred). Room temperature or frozen specimen also acceptable.
Specimen stability:
Room temperature: 2 months. Refrigerated: 4 months. Frozen: 6 months.
Rejection criteria:
Stability limits exceeded.

Clinical significance:
Increased IgM is associated with Waldenstrom's macroglobulinemia, infectious mononucleosis, viral infections, nephritic syndrome, and estrogen therapy. Decreased IgM is found in selective IgM deficiency, Bruton's disease and acquired immunodeficiency.
